Waiver Statement:

My child has my permission to go to LifeCamp by bus on Saturday, December 14th for the annual LifeCamp holiday party. I understand the group will meet at St. Benedict's Prep at 10:00 and leave Newark by bus around 10:15 am and will return by bus at about 1:45 pm. I give my consent to any necessary medical treatment.  Additionally, I waive all claims against LifeCamp, Inc. of any form of responsibility during the above-mentioned trip on December 14, 2024. I also give my permission for my child to be photographed.
